Medical General Claim Advice

In the event of medical claims, you should submit the following to your insurer as soon as possible:

Outpatient
  • A receipt with the name of the insured person, diagnosis, date of visit, itemization of charges and the signature of your doctor;
  • Completed claim form.
In case your insurer does not require a completed claim form, you should write down the name of the policy holder and the policy number on the back of the receipt.

If claiming for Specialist Consultation or X-Ray or Laboratory test, you should also provide a referral letter from your General Practitioner. Normally a referral letter is valid for six months for the same or related illness or disability.


Inpatient
  • A receipt with name of insured person, sex and age, diagnosis of disability requiring the hospitalization and name of surgical procedures performed, date of admission and discharge, itemization of charge.
  • Completed claims form with your doctor's signature and hospital chop on it.
You can give the claim form to your doctor and he/she will fill in the medical part of the report with the hospital chop on it. Charges may apply for filling and chopping the form.
