Medical General Claim Advice
In the event of medical claims, you should submit the following to your insurer as soon as possible:
Outpatient
If claiming for Specialist Consultation or X-Ray or Laboratory test, you should also provide a referral letter from your General Practitioner. Normally a referral letter is valid for six months for the same or related illness or disability.
Inpatient
Medical General Claim Advice
In the event of medical claims, you should submit the following to your insurer as soon as possible:
Outpatient
- A receipt with the name of the insured person, diagnosis, date of visit, itemization of charges and the signature of your doctor;
- Completed claim form.
If claiming for Specialist Consultation or X-Ray or Laboratory test, you should also provide a referral letter from your General Practitioner. Normally a referral letter is valid for six months for the same or related illness or disability.
Inpatient
- A receipt with name of insured person, sex and age, diagnosis of disability requiring the hospitalization and name of surgical procedures performed, date of admission and discharge, itemization of charge.
- Completed claims form with your doctor's signature and hospital chop on it.
