    Most outpatient treatment will be paid for, upfront, by the policyholder. A claims form will then be submitted to the insurance company and the expenses associated with the treatment will then be reimbursed. In some situations your insurance company may have a direct settlement network whereby you will only need to present proof of your insurance at the time of treatment and the insurer will then settle the bill directly with the healthcare provider.

    If you have received outpatient medical care you should submit a claims form to your insurance company within three months after the treatment. The claims forms that you provide should include all expense receipts for the treatment and a doctors note explaining the treatment.
